REQUIREMENTS FOR NEW APPLICANT:

May download application form through the web or please go to nearby travel agencies for appointment
Personal appearance at DFA
Original Birth Certificate from the municipality/city of Birth (LCR)
Birth Certificate (Security paper from the NSO)
(If no record in NSO, applicant must provide CERTIFICATE of ARCHIVES)
Marriage Certificate (Security Paper from NSO, FOR FEMALE ONLY)
IF MARRIED to a foreigner, CFO clearance required
Original ID of applicants, Valid ID’s (If no hardbound ID, please provide HIGH SCHOOL or COLLEGE YEARBOOK or any LAMINATED issued at least 8 years ago)
New NBI Clearance

FOR SENIOR CITIZENS
If no record in NSO, applicant advised for a LATE REGISTRATION or to provide CERTIFICATE from ARCHIVES
Submit original Senior Citizens ID with photocopy

If late registered, submit any 2 of the following documents with place and date of birth
Baptismal Certificate
Income Tax Return (Old)
School Form 137 or Transcript of Record
Voter’s Registration Record
Confirmation
Old SSS-E1 Form
Join Birth Affidavit (applicable for 65-yr old applicants only)

FOR MINOR APPLICANTS 7 YRS. OLD AND BELOW
Mother will fill out the form
Personal appearance of mother at DFA

If the parents of applicant are not married, provide AFFIDAVIT OF ILLEGITIMACY

SPA (Special power of Attorney) - must be authenticated by the nearest Philippine Embassy of Consulate general if not executed before a Consul designating the representative by name and authorizing him to apply for a passport on behalf of the minor
Affidavit of Support and Consent- must be authenticated by the nearest Philippine Embassy or Consulate General if not executed before a Consul
Original and photocopy of DSWD Clearance
Valid passport of guardian

BROWN PASSPORT

Old passport and photocopy of passport PAGES 1, 2, 3 and the pages showing latest Bureau of Immigration departure and arrival stamps
Photocopy of any supporting document with complete middle name.
Marriage Certificate (Security paper from NSO, FOR FEMALE ONLY)
IF MARRIED to a foreigner, CFO clearance required

GREEN PASSPORT

REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORT

The applicant will appear twice at DFA
Affidavit of Loss
Police Report (if passport is still valid)
Submit same documents required for FIRST TIME PASSPORT APPLICANTS




Please take note that there will be a 20- working- day clearing period for the processing of applicant for the replacement of a LOST VALID PASSPORT

PAANONG ANG MAKULAY AY NAGING MASALIMUOT?

Linggo na naman, masigla akong gumising. Tunay na pag gusto mo at nanggaling sa kaibuturan ang iyong ginagawa ang lahat ng oras ay mahalaga. Maaga ako ngayon at siempre dahil hindi gahol sa oras hindi na ako nakipagpaligsahan pa sa mga sasakyan. Kayat damang dama ko ang kapayapaan, ang sarap ng pakiramdam. Ah! Ito ang "Still in the Bottle" isa sa titulong ipininta ng aking Propesor sa Sining. Sa bukana ng museum nanduon na naman si Maestro. Aba! totoo nga ang sabi niya "moody" ang mga manunulat. Pagkakita niya sa akin ang luwang ng ngiti at siya pa ang unang bumati sa akin. Mausisa siya, Ah! naghahanap ng bagong materyal. kung noong isang linggo tabako ang kanyang materyal ngayon naman ay tasa ng kape na nakalagay sa platitong kulay malaluntian terno sa damit niyang kulay lila. Wow! "harmony color" ang mga kulay na ito ang kulay ng kasaganaan na kasinghalaga ng US green dollar at 100 peso bill.

Nagsimula na si Maestro "ang ating paksa ngayon ay lumikha ng problema at gawing makatotohanan ang hindi totoo?" Tama ba aba mali yata 'yon dahil bata pa ko sinabihan na akong huwag magsinungaling pero eto sya tinuturuan ako ng salungat? Itutuloy ko pa ba? Sige na nga nadito na ako. Ayon naman pala eh, paksa palang problema na pero bilang manunulat hayaang________________ ang mga mambabasa. Ang saya ng buhay! Subalit paano kung ang problema ay iyong sariling mukha? Ano kayang magandang sagot dun? Ha! ang sakit sa ulo. Sa kaiisip? Hindi nauntog kasi ako sa bakal ng kuntador. "Ah! magpaliwanag ka sa Barangay." "Ako pa ang magpapaliwanag ako na nga ang nasaktan". "Kalabanin ba naman ang bakal." "Ano bang gusto mo away?" "Sabi ng di ko nakita" "Anong tawag dyan, bulag?" "Ah hindi malaking "K" "sige na panalo ka na matapos lang eto." "Meron pa akong lilinawin dahil kinalaban mo ang bakal sino ang mas matigas bakal o ulo mo?" "Matiim tiningnan ang kausap nagpupuyos ang galit sabay tayo . Ang dating malamlam na kulay ay naging kulay pula na nagbabadya ng digmaan sa gitna ng pusikit na dilim.

WHAT'S IN STORE FOR US ?

Dream Big, Win Big its the Kerygma Conference year 3 and it was held at the BIG DOME. Last year was really an amazing experience. I saw Ms Ai Ai de las Alas not being a comedian but a renewed Christian who confidently share God's love to everybody. The Big Dome became a place of Big Dreams. The most memorable part is the Healing Session where Mr. Bob Canton lead the crowd. I saw with my own eyes how the old woman on a wheel chair walked and pushed her own wheel chair as Bod Canton ride on it. Wow , amazing ! I must also experience it, but how? I'm on the upper box while it is on the Patron A (lower box). I wore skirt that time but wow! how faith made me jump the hurdles in order to experience that healing power. I f I have no friend who push and encourage me and said "you can make it" They cheer me up, that's a big factor, they are the fuel that light the fire within me. I feel blessed and I wanna share it with everybody.
