PASSPORT REQUIREMENTS?

REQUIREMENTS FOR NEW APPLICANT:

May download application form through the web or please go to nearby travel agencies for appointment
Personal appearance at DFA
Original Birth Certificate from the municipality/city of Birth (LCR)
Birth Certificate (Security paper from the NSO)
(If no record in NSO, applicant must provide CERTIFICATE of ARCHIVES)
Marriage Certificate (Security Paper from NSO, FOR FEMALE ONLY)
IF MARRIED to a foreigner, CFO clearance required
Original ID of applicants, Valid ID’s (If no hardbound ID, please provide HIGH SCHOOL or COLLEGE YEARBOOK or any LAMINATED issued at least 8 years ago)
New NBI Clearance

FOR SENIOR CITIZENS
If no record in NSO, applicant advised for a LATE REGISTRATION or to provide CERTIFICATE from ARCHIVES
Submit original Senior Citizens ID with photocopy

If late registered, submit any 2 of the following documents with place and date of birth
Baptismal Certificate
Income Tax Return (Old)
School Form 137 or Transcript of Record
Voter’s Registration Record
Confirmation
Old SSS-E1 Form
Join Birth Affidavit (applicable for 65-yr old applicants only)

FOR MINOR APPLICANTS 7 YRS. OLD AND BELOW
Mother will fill out the form
Personal appearance of mother at DFA
Original Birth Certificate from the municipality/city of Birth (LCR)
Marriage Certificate of Parents (Security Paper from NSO)
If the parents of applicant are not married, provide AFFIDAVIT OF ILLEGITIMACY
Valid passport of mother

If the minor applicant is late registered, submit the following documents with place and date of birth
Baptismal
School Record

If the minor applicant is out of the country, please provide the following additional requisites:
SPA (Special power of Attorney) - must be authenticated by the nearest Philippine Embassy of Consulate general if not executed before a Consul designating the representative by name and authorizing him to apply for a passport on behalf of the minor
Affidavit of Support and Consent- must be authenticated by the nearest Philippine Embassy or Consulate General if not executed before a Consul
Original and photocopy of DSWD Clearance
Valid passport of guardian


REQUIREMENTS FOR RENEWAL OF PASSPORT

BROWN PASSPORT

Old passport and photocopy of passport PAGES 1, 2, 3 and the pages showing latest Bureau of Immigration departure and arrival stamps
Photocopy of any supporting document with complete middle name.
Marriage Certificate (Security paper from NSO, FOR FEMALE ONLY)
IF MARRIED to a foreigner, CFO clearance required
Submit original ID of applicant, Valid ID ex. SSS/TIN/Company ID/Drivers License/PRC/School ID
Personal appearance at DFA


GREEN PASSPORT

Passport and photocopy of inside and back cover plus the pages showing latest Bureau of Immigration departure and arrival stamps (for SEAFARERS, please submit the Seaman’s Book if the latest arrival stamp is shown on it)
Married Certificate (Security paper from NSO, FOR FEMALE ONLY)
IF MARRIED to a foreigner, CFO clearance required
Personal appearance at DFA

For Minor (below 17 yrs old)

Present passport and photocopy of inside and back cover. The pages showing latest Bureau of Immigration departure and arrival stamps.
Photocopy of Marriage Certificate of Parents
Valid passport of mother and photocopy of inside and back cover
Mother will fill out the application form
Personal appearance of mother and applicant at DFA

REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORT

The applicant will appear twice at DFA
Affidavit of Loss
Police Report (if passport is still valid)
Submit same documents required for FIRST TIME PASSPORT APPLICANTS




Please take note that there will be a 20- working- day clearing period for the processing of applicant for the replacement of a LOST VALID PASSPORT



10-15 working days after appearance at DFA- processing period
